Insurance broker Ingo Forsthuber, text: Got your premium letter? Act by 30.11. – KVG 2027 for cross-border commuters

KVG premiums 2027 for cross-border commuters: got your premium letter? Act by 30.11.

The health insurance premiums for 2027 are out – and for many cross-border commuters living in Germany, Swiss basic health insurance (KVG) is getting more expensive again. The good news: you can switch your KVG insurer as of 1 January. What matters is a deadline.

Key points at a glance

  • Deadline: your cancellation must be received by your current insurer by 30 November at the latest – what counts is the date of receipt, not the date of sending.
  • What you can switch: the KVG insurer. As a cross-border commuter, you usually make the choice between KVG and German statutory health insurance (GKV) only once.
  • Only cancel supplementary insurance once the new contract has been confirmed.
  • Pay any outstanding premiums first – otherwise a switch is not possible.

How premiums are developing in 2027

Separate premiums apply to cross-border commuters living in Germany. For the insurers in our premium overview, the development for adults (aged 26 and over, without accident cover, deductible (Franchise) CHF 300, per month) looks like this:

Insurer20262027Change
SympanyCHF 227.90CHF 243.70+ 6,9 %
SWICACHF 241.80CHF 252.00+ 4,2 %
HelsanaCHF 352.50CHF 358.10+ 1,6 %

The difference between insurers can be several hundred francs per month. You can find all values – including for children, young adults and with accident cover, as well as a market comparison of all insurers – in our premium overviews 2025 to 2027.

The most important deadline: switching as of 1 January

If your insurer notifies you of a new premium, you can cancel your basic insurance with one month's notice to the end of the year. The new premium must be notified by 31 October at the latest. Your cancellation must then have been received by the insurer by 30 November. Cancel in writing, ideally by registered mail, so that you can prove receipt.

What you cannot switch

The premium increase does not open the way back into German statutory health insurance. As a cross-border commuter, you usually make the choice between KVG and GKV only once – at the start of your employment in Switzerland. You can, however, switch your KVG insurer. Caution with supplementary insurance

Basic insurance must accept every person – without a health check. This does not apply to supplementary insurance: here, health questions may be asked, and an application may be rejected or accepted only with restrictions. So only cancel your supplementary policies once the new contract has been confirmed.

Your checklist

  1. Check your new premium letter: how much is your premium rising, and for which family members?
  2. Review your deductible: does your deductible (Franchise) still match your actual health costs?
  3. Compare offers: compare the premiums and benefits of the relevant KVG insurers side by side.
  4. No outstanding premiums: as long as premiums or cost-sharing amounts are outstanding, you cannot switch insurer.

Frequently asked questions about switching KVG

By when do I have to cancel my KVG to switch as of 1 January 2027?

Your cancellation must have been received by your current insurer by 30 November 2026 at the latest. What counts is the date of receipt, not the date of sending. Cancel in writing, ideally by registered mail.

Can I go back to German statutory health insurance (GKV) because of the premium increase?

As a rule, no. As a cross-border commuter, you generally choose between Swiss KVG and German GKV once, at the start of your employment. You can, however, switch your KVG insurer.

Do I have to answer health questions when switching KVG?

Not for basic insurance – it must accept every person. Health questions may be asked for supplementary insurance. So only cancel supplementary policies once the new contract has been confirmed.

What happens if I still have outstanding premiums?

As long as premiums or cost-sharing amounts are outstanding, you cannot switch insurer. So pay outstanding amounts in good time before the deadline.

What does support from KiS cost?

There is no additional fee for you for our support with comparing and switching KVG. As is usual for insurance brokers, we are remunerated by the insurer.
